The Groundswell Project has created a fabulous tool to help you plan for your final days. "Your Final Checklist" is a wonderful way to get the conversation started and a way to help you plan your end of life journey.
This document includes checklists for the "practical stuff", the documentation you will need, how you want your "send off" to look and there is also a section for topics to discuss with your loved ones like... would you prefer to die at home? Is there something comforting that you might want in the room with you as you are dying? and will look after your pets when you die?
A great document to think about at any stage of your life journey really, as I sit here typing, aged early 40's and in good health I find myself thinking about these questions and realise there is much to consider. Please feel free to click the link to download and print.
